Safety Incident Reporting With AI Assistance

Safety reporting is a legal and moral duty. An agent can unstick a form. It cannot declare an incident closed or decide that nobody was hurt.

The worst safety reports are late, thin, and written to protect a narrative. The second worst are fluent, late, and written by a model that has never stood on the floor. If you add an agent to incident reporting, you are choosing to fight the first failure without creating the second.

Indian plants already sit under the Factories Act and a thicket of sector manuals, OEM procedures, and company standing orders. We will not invent a clause number for you. We will not invent an OISD or DGMS circular. Open the manual that already binds that unit. Then decide what a draft may touch.

What assistance means on a bad afternoon

Assistance is: the official form on screen, fields mapped to the standing order, prompts that ask for time, place, people, equipment identifier, immediate action, and witnesses. The agent can point at a similar closed case's structure, not its conclusion. The supervisor still submits.

Assistance is not: deciding first-aid versus reportable, deciding lost-time, deciding whether to tell the inspectorate, or writing that no unsafe condition was found because the model prefers calm prose.

If your statutory clock for notification is in hours, the agent's value is speed of completeness, not a second committee.

Data that burns if it lands in a trace

  • Names of injured persons and next of kin.
  • Medical detail.
  • Photos of people.
  • Contractor identity documents.
  • Aadhaar or any identity number someone pastes for completeness.
  • Unverified blame.

Keep those fields in the safety system of record. Do not ship them to a model host. Do not put them in an eval set for a vendor bake-off. Redact aggressively if you retrieve prior cases.

Similar cases are a library, not a verdict

Retrieving a prior RCA can stop a repeat. Retrieving a prior RCA can also import a convenient cause. Show the handle and the date. Ban language like this incident is the same.

Cross-unit retrieval follows the same isolation rule as maintenance search. A neighbour's fatality is not a template to paste.

Never close, never notify the state from the chat

Closure is a competent person's act in the safety system. Statutory notification is a competent person's act on the path your manual already names. The agent drafts. The button stays where the inspector expects it.

If the model is down, the paper form remains. Test that on a drill, not on a real burn.

Objections you will hear — and what to do with them

These are the lines that stall the file. Answer them in the room, then put the answer in the note.

Auto-severity will improve statistics

Statistics that a model wants to look good are how plants lose their licence to be believed. Humans classify against the manual.

We need the agent on the panel to time-stamp tags

Time-stamps can come from a one-way copy designed by OT. The agent still stays off OT. Do not trade a tag for a write path.

Lawyers want every draft kept forever

Drafts can be personal data and can be misleading. Set a retention with counsel. Do not keep every token because someone said discovery. Purpose limitation still exists.

Contractors will not use our system

Then the standing order must say who files when a contractor is hurt. A parallel WhatsApp group is how facts die. Do not add a second unofficial bot.

Four weeks beside the safety officer, not the GPU

  1. Week 1: sit with the safety officer. Copy the live form and the statutory clocks from the manual that actually applies. No invented citations.
  2. Week 2: build a draft UI that refuses empty mandatory fields and refuses medical inference. Keep PII in the safety system.
  3. Week 3: retrieve only redacted closed cases from the same unit. Test a planted same-as-last-time query and demand a refusal.
  4. Week 4: drill a paper fallback. Write the never-close, never-notify, never-OT standing order. Only then allow production drafts.

How this shows up in the file

Subject: Agent assistance for safety incident forms.

The agent may help complete the official form and retrieve redacted prior cases from the same unit. It shall not classify statutory severity, close an incident, notify an authority, or connect to OT. Inferred medical or blame language is prohibited.

Empty is better than calm

Safety prose has a temptation toward reassurance. Models share it. Configure the form so medical and blame fields start empty and stay empty unless a human types. A blank that an inspector can see is more honest than a fluent lie.

Drills are the acceptance test

A tabletop on a Tuesday with a paper form and a 'model down' card will tell you whether the standing order is real. If people reach for the chat anyway, the UI is too tempting and the paper path is too far. Move the paper pad back to the desk. Acceptance is not a fluency score on last year's incidents.

Blame language is a second incident. 'Operator error' in a draft that an officer forwards is a labour-relations event and sometimes a legal one. Keep cause fields empty until the competent investigation says otherwise. The agent may ask 'what did you see', not 'who failed'.

If a contractor is hurt, the standing order already names who files. Do not open a parallel contractor bot that 'makes it easier'. Parallel bots are how two different times enter two different systems and the inspectorate believes neither.

How this clears vigilance and the board

A P1 CIO/CTO in a PSU will meet CVC-shaped questions even when there is no special 'AI circular'. “Safety Incident Reporting With AI Assistance” has to survive a technical committee, a cost centre, and a union conversation if jobs appear threatened.

Questions this usually raises

Does the Factories Act mention AI?
Do not expect it to. It still expects reporting and competent persons. Read the Act and the rules that apply to your factory with counsel. This page does not replace that reading.
Can we use photos of the scene?
Scene photos that do not identify people may be useful in the safety system. Do not dump them into a general model index. Faces and injuries stay out of traces.
Should near-misses use the same agent?
They can use the same form-completeness help if the standing order already treats near-misses seriously. Do not let the model downgrade a real incident into a near-miss.
What if the vendor claims a certified safety model?
Ask which Indian statutory duty that certificate discharges. If the answer is none, it is marketing. Keep the human on the form.
Will you invent an OISD or DGMS circular number?
No. Open the manual that already binds that unit. Sector instruments change. This page is not a citation factory.
